If you want to count it as a third, the next test was to acquire Zangetsu's bankai, and was brought about by Yoruichi. This occured right before Rukia was to be executed, and took place in a special training area of Yoruichi's.

It might be a pointless argument on my behalf, but that's how I feel. Zangetsu was Zangetsu even before Ichigo learned his name. The question's vague nonetheless. It would've been better if it was clear they were asking about Zangetsu as just a Zanpakutou, or Zangetsu as "Shikai" form. |
