For Kojima himself, pretty sure you'll need to beat atleast 5 Main Missions... considering I've only beaten 7 Main Ops, and I have him...

How chapters it has? It looks like it's far from over.

Portable Ops is non-canon btw. I think the official stance is that it's a telling of events that happened in the canon universe but the telling itself is not 100% accurate (like someone made a movie out of the events and took creative liberties). I think that was the official Kojima stance on it.

I think I got him after mission 4... together with the ability to expand Mother Base.

The true ending episode of The Phantom Pain was not shown in the final game as Konami simply want to rush shipping this game before Kojima managed to finish it. Only available in the collector's edition disc as unfinished ending. Sorry if this will bother you all while playing this game.

I'll refuse to watch this video until I finished the game myself, but from what I heard many hope for a True Ending DLC.
Kojima is still around till December, he actually could finish it if Konami lets him.

But most people fear we will only get Camo, Weapon and Skin DLCs and such...

Like Fox Die it mutated into some kind of zombie strain at mother base in one mission Big Boss kills to his own unit and RD team, yes you will need to re-recruit again. Quiet, blamed herself and left. Big Boss followed they were attacked by Skulls another mission, Snake got poisoned and she ultimately had to speak in English in order to guide the troops to save him. But as a result she went deep in the desert and died because she spoke English. Beating up Hideo in MB because I don't want to play as Venom Snake! __________________ Vampire+Sisters

Maybe he got the parasite through Eli, who got it from Tretij in chapter 1 ending. Probably as his "research material" in exchange of repairing ST-84 for Eli. Huey must be blackmailed by Eli. Not only that, he did a lot worse things back in the past. He was accused for the destruction of the MSF by bringing XOF into the old Mother Base, with the inspection team as a disguise. He made a deal with Cipher for that in exchange of him to be spared. And the worst part, he tried to make his son, Otacon, a guinea pig for the ST-84. His wife enraged and hid his son from him. In revenge of her action, he locked Strangelove inside the mammal pod and left her to die inside. If he think he's innocent, then why all of those evidences blew in front of his face? Then this come to a conclusion that there's something actually wrong about him, a deluded madman who's too blind to see his own crime.
